Attach files to questionnaire responses (for vendors)

You can attach one or more files to individual questionnaire responses to provide supporting evidence or documentation. This is useful when a question requires multiple documents to fully substantiate your answer, such as a policy document, a compliance checklist, or a supporting report.

Attached files are visible to all participants in the assessment and appear on the assessment's Files tab for centralized review.

Attach files to a response

To attach files to a questionnaire response:

  1. Open the assessment and click the Questionnaire tab.

  2. Navigate to the question you want to attach files to.

  3. Drag and drop files in or click to browse files. You can upload one or more files.

The attached files appear in a list beneath the question, showing the file name and the user who uploaded them.

Preview, download, or remove attached files

After attaching files, you can manage them directly from the questionnaire:

  • Preview a file: Click the eye icon to open a preview of the document.

  • Download a file: Click the download icon to download the document.

  • Remove a file: Click the trash icon to remove it. You'll be asked to confirm before the file is removed.

Where attached files appear

Files attached to questionnaire responses are also available on the assessment's Files tab, where assessors can review all assessment documentation in one place.
